Sacrament of Confirmation Preparation

(Please note: Families registering children and youth in Faith Formation and Sacramental Preparation programs must be registered and active parishioners of the Basilica of St. Paul Catholic Church.)

Our Parish Faith Formation team seeks to work in a collaborative effort with parents to “reveal the mind and heart of Jesus,” through our religious instruction, sacramental preparation and spiritual formation offered to the children and families. 

INSTRUCTIONS TO REGISTER FOR FAITH FORMATION Grades Pre-K4-7:

1.     Download and complete the Faith Formation Registration form. 
 Forms will also be available in the Narthex (lobby) of the Basilica.

2.     The Registration Form may be completed and scanned in and emailed to manager@stpauldaytona.org or the Registration Form may be returned to the parish office during office hours.

3.     If your child is receiving First Holy Communion then please include a copy of your child’s baptismal certificate with your registration form. If your child was baptized at the Basilica of St. Paul we will provide the baptismal certificate.

4.     Download and complete the Parental-Guardian Medical-Information-Consent form and return it with your parish registration form.

5.     Download the Faith Formation Calendar for your reference.

6.     The Registration Fee is $20 and may be paid by cash or check in person at the office, mailed in, or provided at the first evening of class on September 18.

1.     Download and complete the Confirmation Preparation Registration form. 
 Forms will also be available in the Narthex (lobby) of the Basilica.

2.     The Registration Form may be completed and scanned in and emailed to manager@stpauldaytona.org or the Registration Form may be returned to the parish office during office hours.

3.     Please include a copy of your child's baptismal certificate with your registration form. If your child was baptized at the Basilica of St. Paul we will provide the baptismal certificate.

4.     Download and complete the Parental-Guardian Medical-Information-Consent form and return it with your parish registration form.

5.     Download the Confirmation Preparation Calendar for your reference.

6.     The Registration Fee is $50 and may be paid by cash or check in person at the office, mailed in, or provided at the first evening of class on September 18.
(Please note: if your child attends 8^th grade at the Basilica School, this fee was already processed as part of your child’s tuition.)
